These instruments were used in 1999-2000 for a descriptive study of Family Decision Meetings used by Oregon’s child welfare agency. The resulting report was “Family Decision Meetings: A Profile of Average Use in Oregon’s Child Welfare Agency.â€? In addition to forms for a Caseworker interview and a Family member interview, the instruments include: Case fact sheet, Attendance record, Meeting facts, Information about the plan, and form for Who’s involved.
