This set of tools was used in the evaluation of a Manitoba Family Group Decision Making model project’s first phase. The tools include forms for recording referral information, the conference, and review of plans from the conference, in addition to a referral worker survey, family participant post conference questionnaire, and family feedback focus group questions.
